Pinnacle Gastroenterology schedules appointments Monday – Friday. For some insurance purposes a referral may be required by your primary care physician. If you are not sure of your insurance guidelines please call the customer service number on the back of your insurance card. All appointments are made according to the clinic availability.

Required Patient Forms

Save time by printing and competing your new patient information packet before your visit. Other signature paperwork will need your signature but must be competed the day of your appointment in our office.

Fees and Billing

All Co-pays and Deductibles are due at the time of your visit.


We accept Cash, Check, Money order, Mastercard, Visa, American Express and Discover credit cards.

Pay Your Bill Online

You can now pay your bill online. Go to to access our online bill payment system. You will need your account number. Charges will be posted within 48 hours of submission.


Pinnacle Gastroenterology accepts most insurance plans. Your insurance coverage is an agreement between you and the insurance provider . We are required to collect all Co-pays and deductibles that apply at the time of service.

We ask that you provide us with a current ID along with your insurance cards. As always the final responsibility of the clinic charges are the patients responsibility. Failure to keep us informed of any changes in coverage that may occur with your current insurance coverage could result in full patient responsibility.

Your information is privacy-protected. You must sign a request in order for us to release information to your insurance company or to friends / family.
